Feel free to correct me if I'm wrong, but I believe he was on some kind overly restrictive keto diet. At a low bf, not eating carbs is a recipe for a "disaster"., so any side effects he was dealing with were amplified by an unsustainable dietary approach and it was only a matter of time till' he fell off. If he focused on what actually matters while adopting a bit more flexible and sustainable approach, the story would most likely be entirely different.
